Information Coming Home

Hi everyone,

Today (and possibly before, depending on how often your child checks his or her classroom mailbox!), several forms should have come home.  The following are the forms and the dates they need to be returned:

Picture Day form - form needs to be returned with selections marked by October 25th (that is our scheduled picture day); no money needs to be brought in that day.
HPV Vaccine permission form - this is for the HPV Vaccine form was sent home with girls only - it needs to be returned as soon as possible, clearly indicating whether or not you would like your child to receive the shot - I will list the dates once I have them.
October 31st Dance-a-thon permission form - students need to have a signed permission form to attend, however donations are optional. If students do not attend the dance, an alternate activity will be provided for them during the dance time.
